Transitional Care Center
If you’ve had a procedure like hip replacement surgery, or something unexpected like a stroke or cardiac event, you may need transitional care to help get you back on your feet. The Transitional Care Center at St. Ann’s Community provides short-term rehabilitation for residents and other patients, providing:
- Skilled nursing
- Physical therapy
- Occupational therapy
- Speech therapy
- Therapeutic recreation
