Providing highly compassionate care at the end of life is one of the most important things we do at St. Ann’s Community.

Long-term Hospice Care

We offer long-term hospice and palliative care in our Leo Center for Caring, a quiet, 10-bed unit nestled in its own space on our Irondequoit campus. Admissions are accepted from the greater community. The Leo Center  provides a peaceful atmosphere that ensures dignity, sensitivity and comfort. It is an environment of warmth and caring that helps patients complete their lives with dignity, meaning, and peace. Our specially trained clinical staff is skilled at providing palliative (comfort-oriented) care, allowing patients and their families a special opportunity for peace and closeness.

Hospice Care for Our Nursing Home Residents

To meet the needs of our terminally ill nursing home residents, St Ann’s partners with licensed and certified hospice agencies to provide care that assures comfort and dignity at the end of life.

Families, residents and patients in all of our communities have the right to choose the hospice agency they want to provide their care. St. Ann’s has relationships with both Visiting Nurse Service Hospice and Lifetime Hospice. Both agencies provide hospice care to people in their homes, including independent living, assisted living and skilled nursing on both our Irondequoit and Webster campuses.
