Home Connection
A medical adult day program designed for participants who need personalized health support in a comfortable, stimulating setting. Each participant receives an individualized care and recreation plan developed by our interdisciplinary team in partnership with families and physicians.

A flexible medical and social adult day program open to the community — residency is not required.
