Fall Prevention Support
Keeping your loved one safe every step of the way
Falls can happen to anyone, but many are preventable when risks are identified early. At St. Ann’s Community, we take a proactive, personalized approach to fall prevention — supporting both safety and independence.
Our Approach to Fall Prevention

We work with families and care teams to assess fall risk and create a personalized safety plan.
Therapy Services
Physical and occupational therapy help improve strength, balance, and safe daily movement.
Ongoing Prevention
We continually assess fall risk and adjust care plans to support both safety and independence.
Who Is at High Risk for Falling?
Your loved one may be at increased risk if they experience:
- A history of falls
- Difficulty with balance or walking
- Trouble standing from a chair or toilet
- Vision or hearing loss
- Memory or confusion challenges
- Dizziness or lightheadedness
- Ongoing pain
How Families Can Help
Stay Connected
Share any fall history or concerns with our care team so we can better support your loved one’s needs.
Encourage Safe Movement
Encourage your loved one to use the call bell for assistance and to stand slowly to help prevent dizziness and loss of balance.
Be Involved
Regular visits can help you notice changes in mobility, strength, or overall health. Please notify staff when leaving so we can maintain safety measures.
