Rehab & Transitional Care
Highly Skilled Rehab Care
After a planned procedure like hip replacement surgery – or an unexpected event such as a stroke or cardiac episode – you may need transitional care to safely regain strength and independence.
At St. Ann’s Community, we provide highly skilled rehab care designed to help you recover comfortably and return home as soon as possible.
Two important things to know:
- You have a choice in where you receive rehab care – tell your hospital social worker you want St. Ann’s Community.
- Preplanning your stay makes recovery easier – you’ll know a room is ready and avoid last-minute decisions during a stressful time.
When you preplan, our team helps arrange everything in advance so you can focus on healing with confidence.

“I first went to St. Ann’s for rehab years ago and I’ve gone back twice for different health issues. I love it there! You couldn’t ask for better care. I hope I’m done needing rehab, but if not, I’m going to St. Ann’s!”
