St. Ann’s Community understands the stress and confusion that can occur when you need to make a fast decision on behalf of a loved one. That’s why we’re here to help and answer your questions.

Be aware that whether your loved one needs rehab/transitional care or skilled nursing, the choice of where he or she goes for care is up to you. No one—not even the hospital—can dictate where your loved one should go.

If your loved one is currently at the hospital and needs rehab/transitional care or skilled nursing, tell the hospital that you are considering St. Ann’s and that you want their clinical information, called the referral, sent to us. This is a very important first step and critical to do as soon as the hospital tells you this is the type of care your loved one requires. The sooner you have their information sent to us, the sooner we can begin working with you.
